The following documentation will be required to finalize your child’s enrollment in the Highlands Ranch Mountain Lions program. We understand that providing this documentation can be time consuming. It is important to be clear that these are mandatory and your child will not be eligible to play/cheer without all of the requirements being met.
These forms will be due prior to the first day of practice.
• 2013 Player Contract – Parent/guardian should thoroughly read and complete both sides of this form. This form must be signed by both the parent/guardian and the player.
• 2013 Physical Form – Parent/guardian should complete and sign Section I of this form. The second page, Section II, is to be completed only by a medical professional and must be dated in the current year (2013). This is the only form that will be accepted by Pop Warner. Copies of standardized health forms, physical exams, etc. are not acceptable.
• Proof of Age – Birth Certificates or current (non-expired) passports are acceptable forms of proof of age. The Birth Certificate must be a copy from the city, town, or state the child was born and it must be signed by the local town/city registrar or clerk and must carry a State Seal. Please bring both the certified copy and a photocopy. The certified copy will be reviewed for verification but will not be kept by the association. The photocopy will be retained with all other required documentation. Foreign birth certificates must be translated to English and must be notarized. Court documents for name change and/or adoption are required when applicable.
• Report Card – A copy of the child’s current year school report card is required and must show the full 2012-2013 school year, all trimester/quarter grades, name of school and child’s name. This must be shrunk to fit on an 8 ½ x 11 sheet of paper (may be 2 sided if necessary).
